Position Type: Part Time, Temporary
Hours of Work: 17.5 hours/week
Office Operation Hours: Monday/Tuesday/Friday 9am – 5pm, Wednesday/Thursday 9am – 8pm
Salary: $28.70-38.18/hour
The Family Counselling Centre is an established community-based agency providing counselling services and supporting individuals with developmental disabilities, mental health challenges, and complex care needs. We provide inclusive, person-centered services that promote independence, dignity and quality of life to Sarnia and Lambton area since 1955.
We are seeking a compassionate and skilled Complex Special Needs Worker to support individuals with high needs in daily living, behavioral management, and community integration. The worker will provide service coordination for individual cases where complex needs have been identified. Funds are to reduce safety risks to the child and family members. This role requires a trauma-informed approach, strong interpersonal skills, and the ability to work collaboratively with families, clinicians, and support teams.
SPECIFIC RESPONSIBILITIES:
· Provide planning and support to individuals with complex physical, developmental, and behavioral needs
· Support with individualized care plans, including behavior support strategies and therapeutic interventions as per CSP service plan
· Work as a navigator for the case as outlined in the CSP service plan while receiving CSN funding Foster a safe, respectful, and inclusive environment
· Monitor, plan, and document budget and progress, incidents, and changes in behavior or health and report to MCCSS when needed
· Support community participation and social inclusion
· Complete any additional duties, not outlined above, as deemed necessary by the Programs Manager and/or Executive Director.
KNOWLEDGE, SKILLS AND ABILITIES:

· Always exerts a professional and respectful etiquette.
· Excellent attention to detail; to document and monitors interactions of clients
· Proficient with computer systems and familiar with MS Word, Excel, Adobe
MINIMUM QUALIFICATIONS:
· Diploma or degree in Social Services, Developmental Services, Psychology, or related field
· Experience working with individuals with complex needs, including dual diagnoses and behavioral challenges
· Knowledge of trauma-informed care, crisis intervention, and positive behavior support
· Valid driver’s license and access to a reliable vehicle (if applicable)
· Bilingualism is an asset
